Envision LIFE
LIFE (Living Independently For Elders) St. Joseph of the Pines has submitted an application to the Centers for Medicare and Medicaid to become a Program of All-inclusive Care for the Elderly (PACE) provider. Medicare, Medicaid or private payment are funding options for eligible participants.
Eligible participants are:
- 55 yeas of age or older
- Live in the designated service area
- Are eligible for Medicaid nursing facility care by state requirements
- Can live safely in the community with PACE services
